Medical PANCE Exam (Physician Assistant National Certifying) Detailed Information

PANCE ELIGIBILITY REQUIREMENTS
To be eligible for PANCE, you must graduate from a program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) as a Physician Assistant Program. PA program graduates will be deemed to have graduated from an accredited program if their PA educational program was accredited at the time of their matriculation.
**Even if you have earned a medical degree from another country, you must still graduate from an accredited PA program to take PANCE.

LIMITATIONS ON ELIGIBILITY: THE SIX-YEARS-SIX-ATTEMPTS RULE
Individuals who have never been certified and who graduated from an ARC-PA accredited physician assistant program on or after January 1, 2003, will be eligible to take PANCE for up to six years after completing the requirements for graduation from that program. During that six-year period, PANCE may be taken a total of six times. When either the six attempts or six years are exhausted, whichever occurs sooner, the individual loses eligibility to take PANCE. The only way to establish new eligibility is to complete an unabridged ARC-PA accredited physician assistant educational program again.
REGISTRATION
You must submit an application and $475 payment in advance to take PANCE. You may schedule your exam anytime (depending on testing center availability) within the 180 day timeframe established for you based on your expected graduation date and the successful submission of all required materials to NCCPA. The five-hour PANCE exam includes 300 multiple-choice questions administered in five blocks of 60 questions with 60 minutes to complete each block. There is a total of 45 minutes allotted for breaks between blocks and you will be responsible for managing your break time. You will have 15 minutes to complete the PANCE tutorial.
**Before registering for PANCE, be sure to review the policies and procedures.
When registering for PANCE, please consider the following requirements:
NCCPA does not accept applications until 90 days prior to your expected program completion date (as provided by your program director). (As used here, "program completion date" is the date all requirements for graduation are completed as deemed by your program.)
The earliest date you'll be able to test is seven days after your program completion (provided your program director has confirmed your program completion date.) This date will be reflected in your exam application acknowledgement e-mail.
You have 180 days from the beginning of your exam timeframe to take the exam. These dates will be reflected in your exam application acknowledgement e-mail.
You may only take PANCE once in any 90-day period or three times in a calendar year.
There will be no testing between Dec. 18-31, 2016.
If there's any chance that you won't complete your PA program requirements on time, wait until you are confident of your program completion date before registering for the exam to avoid rescheduling fees.

CONTENT BLUEPRINT FOR PANCE & PANRE
The material on NCCPA's certification and recertification exams can be organized in two dimensions:
Organ systems and the diseases, disorders and medical assessments physician assistants encounter within those systems; and
The knowledge and skills physician assistants should exhibit when confronted with those diseases, disorders and assessments.
Please note that while the subject matter covered on the certification and recertification exams is the same, there is some difference in the nature of the questions on those exams. Generally, the questions on the initial certification exam are more specific, while questions on the recertification exam tend to address broader clinical issues. This difference is most apparent in the questions related to applying basic science concepts and reflects the different functions of the two exams.
Note: Because of the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) publication, NCCPA will adapt its content blueprints, disease and disorder lists, and psychiatry-related terminology within test questions to conform to DSM-5 classifications and diagnostic criteria for all of our examinations beginning in 2015, according to the following timeline:
PANCE and PANRE administered in 2015:
In these examinations, psychiatry-related terminology within test questions will include the DSM-5 terminology followed by the DSM-IV-TR terminology in parentheses. Example: illness anxiety disorder (hypochondriasis)

The Practice Exams are designed for PAs who are eligible for an NCCPA examination. PAs should use the practice exam as a tool to assess their relative strengths and weaknesses, and to familiarize themselves with the types of questions they'll see on the actual examinations.
Will a Practice Exam show me how I'll do on the PANCE, PANRE or a CAQ?
The NCCPA Practice Exams are provided for educational purposes only. They are not intended to predict your performance on PANCE, PANRE or a CAQ, and you should not interpret your results as a prediction of your performance. The results may assist you in identifying possible strengths or weaknesses in the various content areas that make up the exams content blueprints. The content blueprints are used as a guideline when items are selected for the Practice Exams. However, because the Practice Exam has fewer items than PANCE, PANRE and CAQ exams, the content areas contain a smaller number of items than on PANCE, PANRE and CAQ exams. For example, approximately 3% of the PANCE content blueprint is comprised of hematology questions. Thus, on the 120-question Practice Exam, there would be only 3 to 4 questions in this content area. It is important to keep in mind that performance on a small number of items is less reliable than with a larger number of items. Your performance is based on the limited number of items sampled in the Practice Exam and should not be interpreted as a predictor of actual performance on PANCE, PANRE or a CAQ exam.
What types of content do the Practice Exams cover?
The content of the Practice Exams are based on the content blueprint for PANCE and PANRE. However, there may be some differences in the exact percentages covered in the content areas on the specific Practice Exam form.
What is the format of the Practice Exam?
Each Practice Exam includes 120 multiple-choice questions divided into two sections of 60 questions each. PAs can choose time intervals based on their needs. Without special accommodations, PAs have 60 minutes to complete each section for a total of two hours of testing time. With the special accommodations option, PAs can choose to do the sections in two 90-minute or two 120-minute sections. PAs should choose the same timing option they anticipate will be used during the actual PANCE or PANRE exam.
How much time is allotted to answer the items on a Practice Exam?
Like PANCE and PANRE, the Practice Exams provide approximately one minute per item, if special accommodations are not indicated. With special accommodations, the PA would have one and one-half or two minutes per question, dependent on the approved special accommodations.
Are the Practice Exam items actually previously used items on PANCE and PANRE?
Yes, these are questions that were used on previous administrations of PANCE and PANRE. However, keep in mind that test questions used on Practice Exams will no longer appear on PANCE and PANRE.
How much do the Practice Exams cost?
Each Practice Exam costs $50, which must be paid by credit card when you apply.
How do I apply for a Practice Exam?
PAs can access the Practice Exam application online by signing in to their personal certification record 24 hours a day, 7 days a week. Once registered, PAs will have 180 days to complete the Practice Exam online.
Are there special computer requirements needed to take a Practice Exam?
When you access the Practice Exam application from your personal certification record, you'll see a list of the system requirements. If you see a green check next to each requirement, your computer will support the Practice Exam program. If you see a red "X" next to any of the requirements, your computer, as it is currently configured, will not support the Practice Exam program; and we recommend you contact your system administrator. Note: MAC computers can be used with Mozilla Firefox.
Do I have to finish the Practice Exam in one sitting?
No. Unlike the actual PANCE, PANRE and CAQ exams, the Practice Exam allows you to stop working on the Practice Exam and return later. You will not lose any time; and when you log back into the Practice Exam, you'll be returned to the last question that you had previously accessed. All responses are saved when you access the next question, so taking a break will not cause you to lose any answers you've already entered
Are answers and/or explanations to the Practice Exam available?
No. The Practice Exams are designed to provide participants with feedback about their relative strengths and weaknesses in the content areas on the exam. Specific information for why a particular answer is correct or incorrect is not provided.
What kind of performance feedback will I receive?
As soon as you complete the Practice Exam, you'll receive a performance profile, which graphically displays your relative strengths and weaknesses in the content areas that make up the examination content blueprint.

Can I get my money back if I'm unable to take the exam after applying for it?
Unfortunately, refunds will not be issued for a Practice Exam.
What happens to my answers if I lose my Internet connection while taking the Practice Exam?
The system records all answers each time you access a question and provide an answer. If you are disconnected from the Internet or logged out of the Practice Exam program, you will re-start the Practice Exam on the same test question that was last seen when you sign back into the Practice Exam.
Can I take the exam un-timed?
No. The Practice Exams are designed to mirror the testing environment and timing constraints of an actual exam. If you need to take a break during the Practice Exam, you have the convenience of being able to log out of the Practice Exam without losing any of your time. When you log back in, you have the same amount of time remaining to finish the block of questions. However, keep in mind that this is not the same for PANCE and PANRE. For the actual PANCE and PANRE, you are not able to take any breaks within a section of the test. If you anticipate qualifying for special accommodations, you are encouraged to take the Practice Exam with the appropriate special accommodations timing parameters.
Can I print the Practice Exam?
No. The only part of the Practice Exam that you'll be able to print will be your performance profile that is available after completing the exam.

    Understanding the NCCPA Blueprint for ">PANCE/PANRE

    Before we begin our exploration of the NCC blueprint for NCENRE, it’s important to understand that no blueprint is exhaustive, and no one can know exactly which questions will appear on the Physician Assistant National Certifying Exam (NCE) or the Physician Assistant National Recertifying Exam (NRE).
    Introduction to the NCC Blueprint: Two Dimensions
    The NCC blueprint for NCENRE was developed to provide an overview of the topics, knowledge, and skill areas that appear in the exams. There are two dimensions to the NCC blueprint to aid you in preparing your study and review strategy for NCENRE. Each blueprint outlines the following elements, which are covered in both exams:
  • Organs and organ systems, as well as the diseases, disorders, and medical assessments of these systems.
  • The knowledge and skills a typical Physician Assistant will require in order to practice medicine, regularly encountering those diseases, disorders, and assessments.
  • NCC Blueprint for Content: Organs, Organ Systems, Diseases, and Disorders The NCC blueprint concerning organs, organ systems, diseases, and disorders is fairly straightforward; the table below contains a list of the types of organs and organ systems that will be covered, as well as the approximate percentages of exam content that will be devoted to each topic.

    For the certification and recertification exams taking place in 2015 and beyond, s should be aware of the following changes from the NCC:
  • content blueprints
  • disease and disorder lists
  • psychiatry-related terminology as it appears in exam questions
  • The Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5), which was published in 2013, contains revised terminology for classifications and diagnostic criteria. The NCENRE exam questions, content, blueprints, and other related materials will be updated accordingly, over the course of 2 years in a two-step process:  If you are taking NCENRE in 2015, you will see exam questions that include the DSM-5 terminology followed by the previously-accepted DSM-IV-TR terminology in parentheses when psychiatry-related terminology is called for.  For example, you would see the following listed: illness anxiety disorder (hypochondriasis).
    If you are taking NCENRE in 2016 and afterward, the previously used terminology will not be included. All psychiatry-related terminology within exam questions will follow the DSM-5 guideline. For example, you would see only the following listed: illness anxiety disorder.
    Integration with the NCC Task Area Blueprint
    NCENRE test not only your knowledge of organs, systems, and diseases, but also your knowledge of the essential tasks a Physician Assistant performs in the daily practice of medicine.  The NCC conducted intensive analysis of a Physician Assistant’s practice to determine the knowledge and skill areas that a would need to practice medicine safely and successfully. Therefore, each question on NCE and NRE will address both an organ system and a task area.
    These tasks include:

    Below is a brief summary of each task area so that you understand what will be expected of you on the certification and recertification exams.
    History Taking and Performing Physical Examinations explores your ability to ascertain a patient’s pertinent historical information and risk factors for certain medical conditions, as well as signs and symptoms. You will be expected to know techniques and findings for physical exams, specifically which physical examination techniques are appropriate for selected medical conditions. The cognitive skills for this section mirror the task and include conducting interviews and physical exams, identifying pertinent information from history and physical exam, and associating current complaint with presented history and symptoms.
    Using Laboratory and Diagnostic Studies examines your ability to determine the proper indications for initial and subsequent diagnostic or laboratory studies and procedures, taking into account issues such as cost effectiveness, relevance, normalabnormal diagnostic ranges, potential risks, and adequately informingeducating the patient.  You will need to demonstrate cognitive skills in using diagnostic equipment safely, selecting the right studies or procedures, collecting specimens, and interpreting results.
    Formulating Most Likely Diagnosis tests your knowledge of diagnostics. You will be expected to understand and be able to answer questions about the significance of history, physical exam findings, and diagnostic and laboratory studies, as they relate to diagnosis. Your cognitive skills in normalabnormal diagnostic data correlation, differential diagnosis formulation, and the selection of the most likely diagnosis (taking the available data into account) will come into play for this task area.
    Health Maintenance concerns the epidemiology and early detection and prevention of selected medical conditions as well as how patients can maintain health through lifestyle choices and access to medical care. You must demonstrate knowledge of the relative value of common screening tests, factors that impact health such as occupational environmental exposure, stress, abuse and neglect, and barriers to medical care. You must also be able to educate patients on healthy lifestyles and lifestyle changes, prevention of selected medical conditions and communicable diseases. Immunization comes up in this task area, and you will be expected to answer questions on the risks, benefits, and the recommended schedules of immunization for infants, children, adults, and foreign travelers. Finally, you will be expected to have knowledge of human growth and development, sexuality, issues related to aging, and psychological issues due to illness and injury. Cognitive skills include counseling and patient education techniques, effective communication and bedside manner, adaptability, and ability to research the sources available to you in informational databases.
    Clinical Intervention covers the management and treatment of medical conditions. If treatment includes surgical procedures, you will be expected to demonstrate knowledge of indications, contraindications, complications, risks, benefits, techniques, and technical expertise for selected procedures, as well as sterile technique, standard precautions, special isolation conditions, hospitalmedical facility admission indications, and discharge planning. There will also be questions concerning therapeutic regimens (specifically follow-up and monitoring); the formulation and implementation of treatment plans (especially issues of patient adherence and participation); counseling, educating, and communicating with patients; connecting patients with available and appropriate community resources; and collaborating with other health professionals and multidisciplinary teams. The concept of medical emergencies appears in this task area: recognizing them and initiating the proper treatment. This task area also includes alternative medicine (its role, risks, and benefits) and end-of-life issues.
    Pharmaceutical Therapeutics tests your knowledge of pharmacologic therapies, including mechanism of action, indications and contraindications, effects and adverse reactions, and monitoring and follow-up. You will answer questions on drug interactions, including potential risks, clinical presentation, and treatment. Drug toxicity will also be addressed in this task area, with attention paid to medical cross-reactivity and recognizing and treating allergic reactions. You will demonstrate cognitive skills including methods to reduce errors in prescription, selecting the appropriate drug therapy for medical selected conditions, monitoring and adjusting regimens, and handling adverse drug reactions through evaluation and reporting.
    Applying Basic Science Concepts examines your ability to  apply your training in human anatomy and physiology, pathophysiology, microbiology, and biochemistry to your practice of medicine. This includes cognitive skills such as correlation of abnormal physical exam and diagnostic findings to a given disease process, recognition of normal and abnormal anatomy and physiology, and the ability to relate pathophysiologic principles to specific disease processes.
    These task areas will appear in each question as they relate to your general knowledge. The task blueprint, with its emphasis on process and cognitive skills, is the vehicle by which you demonstrate your comprehensive understanding of the content blueprint, with its topics and information on organs and organ systems.
    NCE and NRE: Similar Exams, Different Purposes
    It’s important to note here that although the questions on the certification and recertification exams cover the same material, the questions do differ slightly. This is because each of these exams has a different function: The purpose of NCE is to assess basic medical and surgical knowledge for newly trained s, and the certification provides you with the credentials needed to inspire trust as you begin your practice of medicine. Therefore, the questions are more specific, especially as they relate to applying basic science concepts. The purpose of NRE is slightly different: it’s about measuring your abilities in the daily practice of medicine and maintaining the skills and knowledge you learned in training.
    The assumption here is that you now have more clinical experience and specialized knowledge from a daily medical practice, and questions on the recertification exam will cover more general clinical concerns. Therefore, although 60% of the exam is comprised of general knowledge-based questions, 40% of the exam is a practice-focused component. With NRE, you have a choice:  the practice-focused component of the exam is directed to general questions in 3 areas of medicine: adult medicine, surgery, and primary care (which is the “default” option). In this way, you can rely on the knowledge you’ve gained from your clinical experience to focus more of the exam on your practice area, if you wish.

  • Physician Assistant Program Resources: ">PANCE

    If you scored below the national mean in the CKRAT exam (fewer than 137.5 items correct), using a study aide will be particularly important for you.  Although a NCE prep course is part of Senior Seminar, some students need to pursue an augmented study plan before taking the NCE.  Now is a good time to start.  Please contact your advisor for any particular questions you have about your possible need for an augmented study plan.

    Other non-Duke Options (most for a fee!)
    As you prepare for the NCE ( National Certifying Exam), you may want to consider the following options to direct your study and enhance your test taking abilities.
    1.    NCC NCE review - $45
    Once you are registered for NCE (able to do this within 90 days of when you are eligible to take the exam), you will see this option. More information given by DP faculty prior to graduation.
    2.    Physician Assistant Exam Review  -$49.99 (iPadiPhone app)
    With over 4,300 practice questions and answers organized into 18 major categories, this app provides a rigorous review of key concepts and clinical pearls. Test your overall knowledge by answering randomly generated questions from all categories or target areas of weakness by focusing on a specific category. Not sure how to interpret an EKG? Choose the cardiovascular section and review the 282 questions listed there. Need a refresher on drug interactions? Take a look at the 487 questions in the pharmacologytoxicology category.  The content follows the NCC blueprint, distilling key facts and clinical pearls essential for exam success.
    3.   AA NCENRE Review Resources (fee varies depending on resource)
    AA survey thousands of s and lists their recommendations. Resources are also rated by the number of s who used the material as well as the percentage who found the review material useful.
    4.   Kaplan NCE Qbank – $99(1 month access) - $199 (until your test)
    Good for both the NCE and the NRE, the NCENRE Qbank provides over 1,200 exam-relevant practice questions with detailed explanations that discuss why the right answer is correct, but also why all of the distracters are incorrect. Create customized practice tests with immediate feedback by timed conditions or tutorial style; unused questions, incorrect or all available; and organ system and task, or a combination of both. Tests can be broken into 90 question blocks.
    5.    HELP NCE Review- Plans range from $225-$500 depending on length of time 
    All NCC blueprint topics are covered in an online format broken down into sections. 2500+ questions in multiple formats. Mock NCENRE exam with explanations.
    6.    Memorang NCE Flashcards - Some content is free otherwise $59
    6,500+ flashcards curated and broken down into digestible bits for easier memorization. Covers all high-yield concepts and organ systems across 17 chapters. Access content on the web or via app (Apple and Android available).
    7.    NCE Flashcard Study System $62 for flashcards
    Flashcards available to review the hundreds of concepts you must know for NCE. *Though a student reports the sample seems to be more like paragraphs of a book chopped to flash card size. 
    **#3 is recommended by a number of former students & #6 is recommended by medical students.
